Some emails could be caught in a spam folder. so maybe try installing this plugin to fix that by not using bcc. Also this functionality might make it into core someday.

Also know that the subscribe turns into an unsubscribe link. If you are subscribed to the topic, it says Unsubscribe. If you are not subscribed to a topic, it says Subscribe.
To expand, we have users who click on the “Subscribe” link (which then changes to “Unsubscribe” as expected) and then receive one or two emails. When they stop receiving updates, they revisit the topic to find that the link that was showing “Unsubscribe” (indicating that they were subscribed) has now changed back to “Subscribe” (indicating that they have randomly been unsubscribed).
Its this random and unwanted unsubscribe action that is the problem.
The emails magically stopping and topics being automatically being unsubscribed is a weird issue. Only thing I could think is that a user may not know that they are unsubscribing their own topics without knowing, or something or someone has run a process to bulk unsubscribe a users topics.
Does it happen to all subscriptions for the user, or just a few?
Any plugins that modify bbPress’ subscriptions?
Have you done some plugin troubleshooting, to see what may be causing the issue??

You don’t have any plugins like toolkits or stylepacks installed that are automatically changing the tickbox for the notfication when replying to a topic ?

Can you confirm you are not using extra plugins like ‘bbPress Moderation’ ?
