I have seen this more than once and would like to know how to prevent the issue.
First post hopefully the user doesn’t delete before someone here takes a look.
Basically no photo just a block with the photos name.

ok to fix this you need to find some commonality.

so

image size,
image type (jpeg,png etc)
particular users
users with particular role
how often – 1 in 100, 1 in 2?
time of day

etc.

It could of course just be loading timing out due to a user’s computer being slow at that moment, internet slow at time of upload, server backing up, or just a glitch. Computers are like that.

so start by getting user to try again with the same image, that will eliminate lots of stuff if it works second time.
