If you create a new entry for keymaster, make sure in the ROLE column you use the word "keymaster" (without the quotes in lower case).
But for now while it's still not working, please use the "reset all to defaults" feature in the control panel to make sure it's all okay.
I have this tested working on a variety of installs and it's very confusing why yours is not working...

Okay thanks to your help I think I found the problem.
You have the throttle override set for the keymaster (or it may be set by default) and that's confusing my code. I have to program a fix for that.
I'll put up version 1.05 a little later and that should fix the keymaster.

I seem to be having difficulty with the post count and titles. I have changed one member from a member to a moderator, and the change has not been reflected in his title. Additionally, post counts have remained the same, despite the posting of new topics and comments.
EDIT: I did not realize we had to update the post counts manually. However, the moderator title has still not changed.
You do not have to update the post counts manually. That's just a reset in case something gets out of sync (should be rare if not non-existant).
Post counts should increase automatically as well as title colors.
Oh wait, I remember your forum. Are you still running 0.8.2?

Still not working. I haven't upgraded bbpress because I haven't seen the need. Perhaps I should now...
Well I definitely would like to get it working with 0.8.2 because I myself have a big site using that version. If 1.06 isn't doing it, there's still something else I missed. Give me some time to investigate today.
UPDATE:
I've installed 0.8.2.1 as a test install.
1.06 is working properly under it. I see post count increases automatically and custom colors/names for keymasters & moderators.
One thing I need to point out - if you've given someone a custom title under their profile - my routine does not kick in. It will bypass them. So if you typed in "moderator" instead of picking it from the dropdown - it's not going to work.
If you haven't done that, let me know what other plugins you are using and I will check for incompatibilities. I am seeing the css wrapper post_count_plus when I look at the source on your pages so it's definitely running - it's just not detecting the moderator status.
The post count is now working. We updated to 8.3.1.
The title is still not working.

Only way I'll be able to figure this out - I can't reproduce it.
ps. I think you need to delete "fix bbpress" it's meant for pre-0.8.2 versions? Unless you stuck other functions in there. It might even be interfering with something.
UPDATE: here's a diagnostic version you can install to help me:

It puts hidden (safe) info into the pages so I can see what the code thinks it's doing (or not).
I have installed the diagnostic version of this plugin and my Moderator (Roger 525) is not being marked a Moderator.
Apparently if you capitalize Moderator in the settings, it does not recognize it. I changed the m to a lower case and now it's working properly.
Ah ha! I should do a strtolower on those anyway, sorry about that. Thanks for letting me know...

I've posted 1.07 which addresses case/spaces in role names and fixes a minor bug where the column titles in the admin menu were not being displayed after saving.
